The short answer: one visit is possible, but two devices are not automatically better

Ultherapy Prime and Thermage FLX may be considered during one Seoul visit when a physician decides that two distinct tissue goals justify the added treatment. Same-day treatment is not a standard upgrade, and a short itinerary is not evidence that both devices should be used.

Small studies have examined sequential monopolar radiofrequency and focused ultrasound in one session. They support a clinical discussion, but they do not establish one universal Ultherapy Prime plus Thermage FLX protocol for every face, treatment area, line count or shot count.

Ultherapy Prime and Thermage FLX deliver different energy patterns

Ultherapy Prime uses microfocused ultrasound with real-time imaging. Its current US instructions say imaging can visualize dermal and subdermal tissue, confirm coupling and help confirm an appropriate treatment depth. Treatment energy is delivered as discrete focused points at selected depths.

Thermage FLX uses monopolar radiofrequency delivered through a treatment tip while the system cools the epidermis. The FDA 510(k) summary describes 6.78 MHz radiofrequency, impedance-aware delivery and single- or multiple-pass stamping motions. Its energy pattern is not interchangeable with an ultrasound line.

A combined plan therefore needs two maps: which zones and tissue goals belong to focused ultrasound, and which belong to monopolar RF. Adding two full-face templates without explaining overlap, untreated zones and a stopping endpoint is not individualized planning.

What the same-session studies actually tested

A 2018 prospective study enrolled 22 Korean adults and used one sequential session of monopolar radiofrequency and intense focused ultrasound. Follow-up at 20 weeks reported changes in laxity measures and no serious adverse effect in that small cohort. The study does not by itself define a universal branded-device protocol or predict an individual result.

A 2025 split-face study in 24 Korean adults compared radiofrequency followed by focused ultrasound with the reverse order. Both sequences showed clinical changes with minimal side effects; three-dimensional analysis suggested slightly greater wrinkle reduction when radiofrequency came first, while the histology did not show marked sequence differences.

A separate 2019 randomized split-face trial treated one side with microfocused ultrasound with visualization and the other with monopolar radiofrequency at the same visit. It compared the modalities rather than layering both over the same side. Together, these studies are useful but small, device- and protocol-specific, and not proof that every patient benefits from same-day full-face combination treatment.

The sequence result is a clue—not a rule for every appointment

The 2025 sequence study may make radiofrequency-first planning worth discussing, but it should not be converted into a blanket instruction. The abstract reports only 24 adults, short follow-up to day 90 and a slight difference in one three-dimensional wrinkle analysis.

A clinician still needs to identify the exact device, tip or transducer, treatment area, energy settings, ultrasound line count, RF shot count, passes and the reason for treating the same or different zones. Local device labeling and the patient's anatomy take priority over a single small study.

If a clinic cannot explain why two devices are needed, a staged plan may be more informative. Treating once, reviewing the response and deciding on the next step can reduce uncertainty compared with committing to overlapping energy solely because the traveler is in Seoul for one day.

When same-day treatment deserves extra caution or a pause

Tell the physician about implanted electronic devices, metal in the treatment area, open wounds, severe or cystic acne, skin disease, medicines, previous filler, threads, surgery and recent energy-based treatments. The current Ultherapy Prime instructions list active or metallic implants in the treatment area among contraindications and say treatment is not recommended directly over dermal filler.

Thermage's official safety information says not to treat people with a cardiac pacemaker, cardioverter, defibrillator or another electrical implant. It also states that the system has not been studied over skin fillers or in several patient groups. These statements are screening prompts, not a complete online eligibility test.

Pause when the concern may be volume loss rather than laxity, when the treatment zones or previous procedures are unclear, or when there is no practical local follow-up. More energy is not automatically more precise, and individual suitability requires an in-person medical assessment.

Recovery planning matters more when two procedures share one day

The Ultherapy Prime instructions describe temporary redness and mild swelling after face and neck treatment, while Thermage materials describe possible temporary redness or swelling and list infrequent adverse effects such as burns, surface irregularity and sensory changes. Individual experience can differ from the typical description.

Ask which symptoms are expected from each treatment, which would be unexpected, how the clinic will separate causes if a reaction occurs and who can review you after hours. A same-day plan should include records of both devices, treated areas, line or shot counts and the clinician's contact instructions.

For a short Seoul trip, leave enough time for an in-person review before a long flight or important event. Seek prompt medical assessment for severe or progressive pain, blistering, marked swelling, weakness, breathing or swallowing difficulty, or another concerning symptom.

How to compare a combined quote without inventing a bargain

This guide does not compare numeric prices because a like-for-like quote would need to match two exact devices, treatment areas, Ultherapy lines, Thermage shots and tips, clinician, VAT, comfort plan and follow-up. A bundle cannot be compared fairly with two unrelated headline prices.

Ask for each device to be itemized and for the clinic to state whether both cover the same zones or different goals. Confirm what is optional, what happens if the physician recommends only one treatment after examination and whether review is included after you return home.

Can Ultherapy and Thermage be done on the same day?

They may be considered on the same day for a carefully selected adult, but same-day treatment is not an automatic upgrade. The clinician should explain the tissue goals, zones, total energy, sequence and follow-up for both devices.

Which should come first, Thermage or Ultherapy?
