The short answer: it is a reported risk, not a predictable outcome
Ultherapy Prime's official US instructions list fat or volume loss among adverse events reported after market clearance. The same document explains that these reports come voluntarily from a population of uncertain size, so their frequency cannot be estimated reliably and a causal relationship cannot always be established. That means the risk should be discussed honestly, but the label does not show that facial fat loss is common or inevitable.
A 2025 systematic review found 19 peer-reviewed studies covering 506 people. Temporary swelling, redness and pain were the most common treatment-related effects in the published literature; the authors found one published case of subcutaneous atrophy. They also reviewed 106 FDA MAUDE reports, where lipoatrophy and neurologic complaints appeared, while stressing that these reports were largely anecdotal and unverified and cannot provide an incidence rate.
So the useful question is not whether every Ultherapy treatment removes facial fat. It is whether the planned tissue target, transducer, depth, area and line pattern fit your anatomy—and whether a clinician can explain where energy will and will not be placed before treatment begins.

Ultherapy Prime combines focused-ultrasound therapy with real-time ultrasound imaging. If a face is naturally thin, has recently lost weight or already looks hollow, the concern deserves a direct examination rather than a standard line package. Focused ultrasound is not a replacement for missing volume, and more lines are not automatically a better plan.
