Why the ‘not filled’ look is a 2026 conversation
The American Academy of Facial Plastic and Reconstructive Surgery reported a clear shift away from visibly overdone results in its February 2026 release on the 2025 member survey. Patients were described as seeking earlier, subtler and more strategic changes, while neurotoxins and fillers remained among the most common minimally invasive procedures.
That trend is useful only when it is translated into a medical plan. ‘Natural’ is not a standardized dose, a universal face shape or a promise that filler will be invisible. Belotero is a hyaluronic-acid filler family, not one interchangeable formula. Belotero Balance (+), Belotero Volume (+) Lidocaine and Belotero Intense (+) Lidocaine have different product characteristics and US approvals. In 2026, the FDA approved Belotero Volume (+) for selected mid-face volume deficits and Belotero Intense (+) for lip augmentation; Belotero Balance (+) already had named US indications for moderate-to-severe facial wrinkles and folds and infraorbital hollowing.
An approval in one country does not automatically describe Korean labeling, and one Belotero product should not be substituted for another in a price comparison. Before injection, ask for the full product name, sealed box, lot information, expiration date, volume and whether lidocaine is included.

A subtle result comes from restraint, placement and anatomy
Hyaluronic-acid gel can attract water and create smoothing or support, but product rheology is only one part of the outcome. The planned tissue plane, amount per area, facial movement, skin thickness, previous filler and injector technique all matter.
One cubic centimeter is a package quantity—not a recommendation to place all of it in one area. It also does not predict the visible change. A small amount in a thin, mobile area can be noticeable, while a broader structural concern may not be appropriately addressed by filler at all.

‘Natural’ should not erase product-specific boundaries
A product can look soft or flexible in marketing material and still be a prescription medical implant with area-specific labeling. The FDA notes that approvals are based on controlled studies in specified facial or hand areas and that it may not have reviewed every filler for every location.
The current US Belotero Balance (+) information names nasolabial folds and infraorbital hollowing. Newer Belotero Volume (+) and Intense (+) approvals cover different areas and tissue planes. Those distinctions are one reason a traveler should not book ‘one syringe anywhere’ from a menu alone.
Korean availability and authorized uses can differ from the US and Europe. The treating physician should explain why the exact product suits the proposed area, how much is planned and what alternative—including no injection—was considered.
The safety conversation matters more than the aesthetic trend
Common filler reactions include swelling, bruising, redness, pain, tenderness, itching and temporary firmness. Infection, nodules, delayed inflammation, discoloration and asymmetry can also occur. Illness, vaccination or dental work has been associated with delayed inflammation near prior filler in post-market reports.
The most serious risk is unintended injection into a blood vessel. The FDA warns that vascular occlusion can reduce tissue blood supply and lead to necrosis, vision abnormalities including blindness or stroke. Unusual pain, blanching or white-gray-blue skin, vision change or stroke-like symptoms during or shortly after treatment require immediate medical attention.
Ask who will inject, what anatomy and complication training they have, whether an emergency protocol is current and where urgent ophthalmic or hospital care would occur. Hyaluronidase may be part of managing some hyaluronic-acid filler problems, but it is not a guarantee against harm and carries its own risks.
- Disclose every previous filler, thread, implant, surgery and energy treatment—even if years ago.
- Discuss severe allergies, lidocaine sensitivity, bleeding risk, immune conditions, active skin problems, cold sores, pregnancy or breastfeeding.
- Do not proceed while the planned product or emergency pathway is unclear.
Planning Belotero during a Seoul visit
A short Seoul stay can encourage same-day decisions, but filler deserves time for assessment and observation. Build in a buffer for swelling or bruising and avoid placing the appointment immediately before a flight, wedding, photography session or other fixed event.
Bring the dates and product names of prior injections. Ask for a written record of the product, lot, amount, areas, clinician and aftercare, plus a contact route that works after you leave Korea. If you have upcoming dental treatment, illness or another procedure, share that timing before injection.
